Any of the forms below can be printed and filled out at home to bring with you to your first appointment to help save you time!  Have questions please let us know.  All forms available at the front desk on arrival as well- please allow 15 minutes before your scheduled appointment time to fill out the required documents as treatment cannot begin without their completion.
